Use a cloth tape measure or a string and provide a tight measurement at the place where your dog wears it's collar. 
I will add a little bit of slack to the collar.  Please do not measure the dog's existing collar.
   
Would you like a Service Dog Tag or Emotional Support Dog Tag integrated into the collar for an additional $5.00?   These tags can be incorporated into any collar type or weave type.
Would you like an ID Tag Plate integrated into the collar for an additional $5.00?
(You will need to purchase the ID Plate and have it shipped to me.  I will provide you with a mailing address)

Do you need Expedited Shipping? ($5.00 Extra)
It typically takes me 5 to 7 days to have a collar prepared and ready for shipping.  I use the United States Post Office, First Class Mail, which typically takes between 1 to 5 days depending on where you live (I'm in Arizona).   If you need the collar to arrive faster, I can use Priority Mail Shipping which typically takes 2 days to arrive to you for $10.00 shipping instead of $5.00 shipping.
   
Would you like to purchase a bag of dehydrated beef liver for an additional $5.00?
(The beef liver treats are dehydrated in my kitchen and is 100% beef.  Nothing is added.)

Once the Form is submitted, an Invoice will be sent to you via Email from my PayPal account
